In order to provide you the with best evaluation and treatment plan, please fill out the appropriate forms before your first
visit.
After you fill out these forms please bring them with you to your appointment.  Please do not fax or email the forms back
to us.
All of your information will be held strictly confidential and we will not release any of the information to any party without
your permission.
We recommend that you make a list of any questions or concerns you may have and bring this with the forms to the visit.
